The combination of ANZ and Suncorp is likely to make the joint entity stand third largest bank in household lending and household deposits, said the announcement on the NZX.

Suncorp has a large customer base in the home loan segment itself. The business is well established, especially in the home loan segment. According to the terms of the agreement, the CEO of Suncorp Van Clive will report to ANZs CEO. ANZ expects to complete the deal by the second half of CY2023.

The interviews were available for 51 patients who were enrolled in the DCCR Phase 3 program. Casimir Inc (Casimir) completed semi-structured interviews with these participants and interview transcripts were manually coded by trained coders who subsequently analyzed the coded interviews in accordance with a pre-specified statistical analysis plan. Not a lot of space to work out in your studio apartment? Square footage isnt an issue when youre training with kettlebells. All you need is a few feet of space in front of your couch will do quite nicely for an intense kettlebell swinging session. Kettlebells also dont take a lot of room to store. Slip one into the corner of your room and break it out any time you need to break a sweat.

Properly performing kettlebell swings involves squeezing your glutes at the top of each repetition. Youre essentially performing high-volume hip hinges, driving and finishing each rep with your glutes. Research shows that the weight of the bell provides instantaneous feedback as to whether youre sufficiently activating your glutes during swings. (9)

The more efficiently you can activate your glutes, the more successful your big pulls think, deadlifts are likely to be. This may be particularly useful for contact sports athletes who need to brace against tackles or other impacts. Spinal stability also comes in handy during barbell lifts, including when you snatch a loaded barbell very quickly overhead.

The direction of the force on your body during a properly-performed kettlebell swing is largely horizontal, as opposed to vertical. (12) As a result, there may be less shearing force on your spine during kettlebell swings than during loaded barbell lifts, potentially reducing the risk of a low back injury.

With that said, movements like the kettlebell swing have been shown to increase lumbar shear and compression during swinging movements when not performed properly. (13) So make sure youre doing your swings correctly. Keep the bell above your knees, your back neutral, and your lats engaged with a strong hip snap.

Doing kettlebell swings the right way is essential. Heres how to get them done efficiently and effectively.

Keep your chin in a neutral position throughout the lift. Exhale as you explode up to increase your coordination and core engagement.

Its not all about the double-handed swing although that is a tremendously valuable tool to have in your box. These kettlebell swing variations will get your heart thumping and your power skyrocketing, too.

Traditional kettlebell swings are performed with two hands. When you opt for using just one hand, youll be engaging your core even more than normal. Youll need to resist rotation throughout the swing, which requires your core to fire up in a big way.

You can perform this move continuously with one arm at a time. Or, you can alternate your swings, switching hands each time the bell approaches your chest level.

Normally, kettlebell swings are a very continuous, fluid movement. When you add a dead stop, youll be doing pretty much what it says on the tin. Youll be coming to a dead stop on the ground between each rep.

Doing this kills the momentum from rep to rep. Without momentum, youll need to rebrace your body between swings. Youll be generating the force of your initial hike backward from scratch each time. In doing so, youre maximizing your muscle recruitment and therefore, your potential strength gains.

Any time youre staggering your stance, youre putting your body off-balance. Normally with kettlebell swings, your feet will be aligned as normal underneath you. But with a staggered stance, youll be increasing your core engagement and ironing out any strength imbalances you might have between your legs.

It might take you some trial and error to find the optimal staggered stance for you. You want your feet close enough together to maintain perfect, forward-facing form. However, you want them far enough apart to make a difference from your regular stance. Start small and light, and progress from there.

When its time to integrate kettlebell swings into your program, youll want to consider exactly what your goals are. Are you trying to increase your cardio fitness? Practice your hip hinge mechanics? All of this makes a difference in terms of how youre programming your sets, reps, and timing.

After the movement prep components of your dynamic warm-up, kettlebell swings can serve as a great way to get your blood pumping, elevate your heart rate, and prepare your muscles for bigger movements. Perform three sets of 10 to 15 reps with a light to moderate weight toward the end of your warm-up.

Like plyometrics, you can do explosive and maximal force kettlebell swings after strength movements like squats. According to the principles of post-activation potentiation where you follow a heavy strength move with an explosive exercise to maximize your gains this can increase both your strength and power.

Focus here on explosive, low-rep sets. Try following a heavy squat or deadlift set with a set of three to five kettlebell swings.

Using kettlebells to improve your cardio fitness and conditioning is an attractive option for folks who cant stand treadmills. You can program this type of training in any number of ways. You can do kettlebell swings with the Tabata protocol, where youll spend four minutes swinging for 20 seconds and resting for 10 seconds. If youre more experienced, you can take a page out of kettlebell sports book and opt to find out how many crisp swings you can do in 10 minutes.

The possibilities for kettlebell cardio training are nearly endless. You can do a kettlebell swing EMOM (every minute, on the minute), aiming to perform 10 or 20 swings a minute for 10 minutes. Do this or protocols like it as a workout finisher, as its own cardio training session, or even during an active recovery day.

If youve got a kettlebell, youve got the key to improving pretty much any aspect of your strength training. Learning how to swing a kettlebell opens up an entire world of possibilities for your overall cardio fitness and strength. Even if youre a barbell nerd, kettlebell swings will help promote the spinal stability and explosive strength you need to bust through plateaus. Increase in Leptin Levels in Preeclampsia Prompts Cardiovascular Cascade That Puts Mother and Baby at Risk – Diagnostic and Interventional Cardiology

June 30, 2022 Before a baby is ever born, critical supply chain problems with nutrition and oxygen can result in premature birth or even death and increase the child and mothers lifelong risk of cardiovascular disease.

Scientists have found that a midgestation increase in the hormone leptin, which most of us associate with appetite suppression, produces problematic blood vessel dysfunction and restriction of the babys growth inpreeclampsiathat put mother and baby at risk.

Its known that about 20 weeks into a pregnancy, women with preeclampsia experience an increase in the production of leptin by the placenta but the consequences have been unknown.

Its kind of emerging as a marker of preeclampsia, saysDr. Jessica Faulkner, vascular physiologist in theDepartment of Physiologyat theMedical College of Georgiaand corresponding author of thestudyin the journalHypertension.

Leptin, mostly produced by fat cells, is also produced by the temporary organ, the placenta, which enables the mom to supply her developing baby with nutrients and oxygen, Faulkner says. Leptin levels steadily increase in a healthy pregnancy, but specifically what leptin is doing even normally in this scenario is unclear. There is some evidence its a natural nutrient sensor in reproduction or maybe a way to enable new blood vessel growth and/or to stimulate growth hormone for usual development.

But in preeclamptic patients leptin levels go up more than they should, Faulkner says.

The new research looking at the impact shows for the first time that the increase in leptin results in endothelial dysfunction in which blood vessels constrict, their ability to relax is impaired and the babys growth is restricted.

When the scientists inhibited the precursor for the powerful, natural blood vessel dilator nitric oxide, like what happens in hypertension, it pretty much replicates the effect of the midgestation leptin increase.

To make matters worse, the scientists also have evidence that leptin plays a role in increasing levels of the blood vessel constrictor endothelin 1.

Conversely when they deleted the receptor for aldosterone, in this case the mineralocorticoid receptors on the surface of the cells that line blood vessels, endothelial dysfunction didnt happen, saysDr. Eric Belin de Chantemele, physiologist in MCGsVascular Biology Centerand the papers senior author.

We think what is going on in preeclamptic patients is the placenta is not properly formed, Faulkner says. In the middle of gestation, fetal growth is not happening as it should. I think the placenta is compensating by increasing leptin production, potentially with the goal of helping spur more normal growth. But the results appear to be just the opposite.

It can hurt the babys development and increase the risk of long-term health problems for the baby and mother, she says.

While leptin has been associated with preeclampsia, this was the first study to show that when leptin goes up, it induces the unhealthy clinical characteristics of preeclampsia, Belin de Chantemele says.

When they infused leptin in pregnant mice to mimic the surge that happens in preeclampsia, they saw an unhealthy chain reaction with the adrenal gland making more of the steroid hormone aldosterone which could be increasing the production of endothelin 1, also by the placenta.

Their previous work has shown that outside of pregnancy, an infusion of leptin results in endothelial dysfunction. Belin de Chantemeles lab has pioneered work showing that fat-derived leptin directly prompts the adrenal glands to make more aldosterone which activates mineralocorticoid receptors found throughout the body, notably in the blood vessels in females, which is important to blood pressure levels. High aldosterone levels are an obesity hallmark and a leading cause of metabolic and cardiovascular problems.

That work made them hypothesize that the infusion of leptin that occurs midgestation in preeclampsia had a similar impact that deletion of the mineralocorticoid receptors lining blood vessels could resolve. They have connected similar physiological dots in young females in whom obesity often robs the early years of protection from cardiovascular disease that being female typically provides until menopause.

These same players likely are factors in what increases the mothers lifetime risk of cardiovascular problems, Faulkner says.

It means the system is dysregulated and that is basically when you develop disease, she says.

Their goals include better defining the pathways for increased blood pressure and other blood vessel dysfunction, pathways that can be targeted during pregnancy to prevent potentially devastating results for mother and baby, from what Faulkner characterizes as a two-hit condition.

Their findings to date indicate that effective therapies to better protect mother and baby could be existing drugs like eplerenone, a blood pressure medicine that binds to the mineralocorticoid receptor effectively reducing the effect of higher levels of aldosterone, the scientists say.

The problems likely start with the placenta, and potentially inadequate blood flow to the temporary organ early in its development and subsequent failure of the development of the big blood vessels that become the passageway for nutrients and oxygen from mother to baby.

Its known that in preeclampsia there are problems like decreased secretion of placental growth factor. The bottom lines appear to be that by midgestation, the placenta can no longer properly support the baby, which may be why it secretes leptin, possibly in an effort to spur its own growth and normal fetal development, but in reality it contributes to cardiovascular and fetal consequences, the scientists report, including raising the mothers blood pressure.

Preeclampsia rates unfortunately are rising, Faulkner says, both in the number of pregnant women affected and in how severely they are affected. According to an analysis of data from the Centers for Disease Control and Preventionpublishedin January of this year in theJournal of the American Heart Association, rates of hypertension that arise during pregnancy, including preeclampsia and gestational hypertension, have nearly doubled in both rural and urban areas in this country from 2007-19 and have been accelerating since 2014. Gestational hypertension is an increase in a pregnant womans blood midgestation but without associated signs of protein in the urine, a sign of kidney distress, or markers of placental dysfunction, as are found in preeclampsia.

Risk factorsinclude carrying more than one fetus, chronic high blood pressure, type 1 or 2 diabetes, kidney disease, autoimmune disorders before pregnancy as well as use of in vitro fertilization. Increasing rates of preeclampsia are primarily attributed to obesity, which is a risk factor for many of these conditions and associated with high levels of both aldosterone and leptin, Faulkner says. Other times, women seem to develop the problem spontaneously.

Next steps in the research include better understanding how and why leptin goes up more than it should, Faulkner says.

Wong and her colleagues see patients who are having problems with sleep, whether its problems falling asleep, staying asleep, snoringor if the person stops breathing during sleep. They also see patients who move their legs excessively at nightor patients who are excessively sleepy during the daytime.

The following is an edited version of an interview with Wong. There is also anNow You Know Akron podcast of our interview available.

It really depends on your age. If you are 18 and above, seven to nine hours is actually good and when you reach age 65, seven to eight hours is recommended.

However, if you sleep six hours, it's still acceptable. If it's less than six hours, that's probably too little.

There is also a segment of the population whom we call short sleepers. They usually sleep less than six hours and then they feel good. They don't have issues like fatigue in the daytime, problems with memory, mood, concentration or drowsy driving or daytime sleepiness.

For the most part, people usually need seven to nine hours and research has shown that seven is actually better.

There are two stages of sleep: non-REM or non rapid eye movement sleep and sleep when you are dreaming, which is called REM sleep, or rapid eye movement.

Non-REM is usually good for physical health. REM sleep is good for your mental health. During REM sleep, that's the time where our brain is trying to recall what we learned in the morning and in the daytime and then to relearn how to do it.

There are three stages of non-REM sleep before REM. The deep sleep that were talking about or restorative sleep is not really in REM, but Stage 3 of non-REM sleep.

In kids, the deep sleep is very important because thats the time the growth hormone is produced so they can grow taller and all those things.Our immunity is also enhanced during deep sleep.

Yes, so with a normal patient, you start with non-REM stages 1, 2 and 3 and then you go into REM. And then sometimes from REM, you go to Stage 2 and then go back to 3 and then 1. It really depends on the person. So it's like an alternating cycle. Each cycle can last anywhere from 90 to 100 minutes. Each cycle is composed of non-REM and REM.

A good, restful sleep has a lot of benefits for us. Its the time where our memories are consolidated and stored. Its the time where muscles repair and recover.

If you dont sleep well, the following day, you feel like your judgment and concentration areimpaired. Your reaction time is slowed. There's a lot of short-term and long-term issues when youre not getting adequate sleep.

The TV, laptop, cell phone andiPadactually emit blue light. When your brain sees this blue light, it thinks we need to be awake.

We usually tell patients that part of a good sleep habit is to turn off any gadgets that emit blue light anywhere from one to two hours before you go to sleep. We tell patients dont go to sleep unless youre sleepy. If you are in bed and youre not falling asleep in 15 minutes, get out of bed. Go to a chair inside your bedroom or another room and do something boring.

When you get up and grab your phone, youre exposing yourself to the blue light again and its going to be hard to fall back asleep or fall asleep. Do something boring and make sure it's under dim light like yellow light. If you turn on your white light in the room or you start exposing yourself to gadgets that emit blue light, then you start telling your brain thatyou need to be more awake.

Some of my patients flip through magazines or color until they feel sleepy.

You should avoid napping during the daytime. When you're awake during the daytime, you're building up sleep pressure and when the sleep pressure peaks, that's the time you feel sleepy. If you sleep in the daytime, then your sleep pressure goes down and it will be harder for you to fall asleep at night.

Caffeine usually blocks the adenosine in the body and this adenosine is the one that contributes to the sleep pressure.

Different patients have different rates of metabolism. As an example, when I was younger when I drank caffeine at 6 p.m., I could still fall asleep at 9 p.m. But for some reason as I got older, I have to cut my caffeine by noon otherwise I cant fall asleep at night.

As we get older, it seems the metabolism of caffeine is slower. We usually advise patients to cut it off past noon.

I tell patients to limit fluid intake to 8 ounces at dinnertime and after that, do not drink any more water. If you have pills to take at bedtime, make sure to drink it with only sips of water or just enough to swallow the pills. That way, when you go to sleep, you dont wake up a lot to go to the bathroom.

Patients who have some type of abnormal behavior in their sleep would come to see me. Or sometimes its not an abnormal behavior, but breathing problems. The most common patient we see in the sleep clinic is someone with breathing problems during sleep. That can be because the husband is snoring so loud and sounds like a dragon and the wife cannot sleep, so the wifes sleep is affected.

Some patients stop breathing while sleeping or wake up gasping for air or have a choking sensation during sleep. We also have patients who are drowsy driving in the morning, or people who excessively sleep during the daytime.

Some other patients have abnormal sensations in the legs before sleeping. That's usually related to Restless Leg Syndrome or some patients act out their dreams during sleep. Some may have sleepwalking behaviors or some people call them sleep terrors. Those are also things to be seen by a sleep specialist.

Other reasons patients might see a sleep specialist would be problems falling asleep or staying asleep, or fatigue, increased irritability, general malaise or even cognitive impairment, usually in adults.

Adults and kids also show different symptoms. Kids who dont have a restful sleep may show it it in poor school performance, impaired attention or sometimes aggression or hyperactivity.

Still other issues to see a sleep specialist might be narcolepsy, which is not common, but patients can fall asleep while standing or during an activity, like eating. As soon as they fall asleep, they go into the dreaming stage.

Some patients also may have a sleep-eating behavior where they wake up and look for something to eat and dont remember the following day.

Some patients also have circadian rhythm problems, which are body clock issues being awake at night and sleepy during the day.

It depends on the insurance. Some need a referral and some allow a self-referral.

There are two types of sleep studies: a home sleep apnea test and an in-lab sleep study. Sometimes it depends on what insurance will cover. Some insurances only cover an in-home study before they cover an in-lab study. Most commonly, we do sleep studies for sleep apnea and narcolepsy.

An in-lab sleep study will gather a lot more data since there are air-flow sensors, belts watching breathing and monitors on the finger to check for heart rate and oxygen saturation. We also watch heart rhythms and check for leg movement. There are also wires on the scalp to check for sleep staging.

If a patient comes back positive in an in-home sleep study for sleep apnea, there are two options: We either start a patient right away with a CPAP machine (which stands for continuous positive airway pressure) or we can have them come in for an in-labstudy.

If a CPAP machine is not tolerated, we may try a BIPAP (bi-level positive airway pressure) machine or add oxygen. Some patients may need an ASV (adaptive servo-ventilation) machine.

We also sometimes use an oral appliance, worn on the upper and lower jaw during sleep, though it depends on insurance.

Another option is something we call Inspire, which is fairly new. It is basically a battery implanted under your skin that comes with a remote control. At night, the device moves the tongue forward every time the patient is breathing while sleeping. It kind of looks like a pacemaker.

For sleep apnea, theres still research being done to see if there are medications to help. Right now, there are no medications that are highly effective for sleep apnea.

Our body produces melatonin, which calms our body and brain to prepare the body for sleep. Some patients find melatonin supplements effective and some dont. It is not a sleeping pill, but helps the body to prepare to fall asleep.

Sometimes we use melatonin to move the body clock forward if you stay up past midnight, but you have to wake up at 6 a.m. for school, but youre sleepy during the daytime.

Bottom line, if it is for body-clock issues, melatonin is a good medication that we use.

We usually dont want patients taking over-the-counter sleeping pills that have things like Benedryl or diphenhydramine because it can cause you to have restless leg syndrome. We also want patients to sleep naturally and not rely on medications to fall asleep.
